    The pediatric medical home is a family-centered approach to providing comprehensive primary care. In a medical home, healthcare professionals work with patients and their families to ensure all medical and non-medical healthcare needs will be met. In this holistic model of primary care, the pediatric care team helps the family to access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services important for the overall health of the child.
